labour market or remain employed. Submit the completed form in person or by
mail to your
local employment centre.

To apply for last-resort financial assistance benefits, both these forms
must be completed. If the application is for a couple or family, each adult
has to complete an
Application for Service form (3000A); only a single
Application for Last-Resort Financial Assistance form (3000-01A), however,
has to be completed per couple or family. Submit the completed forms in
person or by mail to your
local employment centre.
Important: Your
eligibility for last-resort financial assistance benefits is assessed as of
the date on which the local employment centre receives your forms.

Under the Canada-Quebec Labour Market Agreement, Québec is responsible for
active employment measures available to employment insurance participants,
and for certain functions of the National Employment Service available to
employment insurance users. These measures and functions are funded by the
Employment Insurance Account, for which Canada is responsible. |
